Requirements for Issuing an I-20:

1. Official Copies of School Transcripts of student's grades for at least the past 3 years. (**English Translation)
   These must clearly state each credential, provide U.S equivalent, and provide the U.S. credit and grade equivalent for each subject.
2. Copy of Passport and / or Birth Certificate
3. Bank Statement / Affidavit Support of person responsible for paying tuition and fees. (Proof of financial support is required on I-20 form)
4. Name, Address, and Phone Numbers of Guardian in the U.S. that student will be living with.  
5. Complete Foreign Address
6. At least (2) completed character reference forms in English translation
7. *Copy of all current medical/immunization records in English translation. Record must include all vaccinations from the birth of the student until the current year. All vaccinations must be up-to-date as required by the Houston Health Dept.
8. *Student Health Insurance: Memorial Hall School requires that all International Students have health insurance. Information is available through the Admissions Office. Students may purchase their own health insurance, but must provide MHS proof of insurance.

*After Admission

**Please contact the school office if you have questions regarding the English translation of transcripts. We have a list of available certified agencies. No students will be issued I-20's without complete transcripts being received by Memorial Hall.

***REQUIREMENTS F0R INCOMING SENIORS FOR 2012-2013 SCHOOL TERM***

In compliance with the recent changes in the requirements from the State of Texas, it may be necessary for incoming seniors to study for up to 2 years in order to receive all required credits for graduation. It is of the utmost importance that all transcripts be officially translated and submitted before any graduation date can be determined.

No transcripts will be accepted after September 1, 2012 for students studying in the Fall - 2012 Semester. No transcripts will be accepted after January 15, 2013 for students studying in the Spring - 2013 Semester. Any student failing to provide a certified translated transcript will be placed in the eleventh grade in order to complete all required credits.

***There will be a $600.00 non-refundable processing fee for each I-20 form issued by Memorial Hall School.

ISSUANCE OF I-20 - The I-20 document may be picked up by the guardian. There will be a $50.00 fee if Memorial Hall needs to send the documents by FedEx.

****Sevis I-901 REQUIRED FEE - $200.00****

All applicants applying for the F-1 Visa must also pay the I-901 Fee. This fee is paid to the U.S. Govt. It can be paid on-line - www.fmjfee.com. The I-901 fee must be paid and the family must have the receipt for payment before going to the U.S. Consulate for their interview. The student must have a SEVIS I.D. number (found in the upper right hand corner of the I-20) before the fee can be paid. For more information visit the SEVIS website (http://www.ice.gov/sevis).

All students receiving their F-1 Visa's will be required to complete no less than three complete concurrent semesters with Memorial Hall School.  Annual Tuition and fees for these students must be paid in full and are non-refundable.
